Your zip file should look something like this "JaneSmith.zip".

 

Please include the following:

1. A Current Work Resume (with contact information).

2. A Photo of a Government Issue I.D. (18 or older only).

3. A Head Shot of your self (no duck kissy faces please).

4. A Full Body Shot (revealing but not nude).

5. And please, please, please...know how to dance!
